dc.description.abstractMillions of the elderly and people with disabilities around the world suffer from difficulties in carrying out their daily life tasks. However, thanks to the continuous technological development that affected all fields, including the medical one, it became possible to integrate them in a greater way into society and facilitate their lives, even partially. To do this, in this project we took advantage of the Internet of Things technology (iot) to collect measuerments of their vital signs through various sensors, and to gain interoperability, we needed semantic web to make the data more understandable to the machines, by using many of its technologies (ontology, SWRL, etc). We also used fuzzy logic in an effective way to resolve disputes and uncertinty and for better evaluation of options. The combination of these disciplines and technologies produces an ontological model for us that contains various informations about the target person (type of disability, vital signs, objectives and healthcare services he needs, etc.) to facilitate doctors' follow-up to them and make their life easieren_US
